Nurse Practitioner - PRN

As a Nurse Practitioner – PRN, you will conduct and document comprehensive health assessments (CHA) during a visit that can last up to one hour. Your assessment will review the member’s current health, medical history, medication adherence, social environment, and other risk factors, allowing us to close multiple care gaps with a single visit.
Responsibilities
Conduct adult/geriatric assessments to include medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation in accordance with Matrix guidelines and protocols.
Administer and order point‑of‑care and lab‑based testing as needed.
Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary personnel.
Work with the Primary Care Physician (PCP) on patient education and follow‑up as necessary.
Provide services at the member’s place of residence, which may be in their home or a skilled nursing facility.
Benefits
Flexibility – Visits are scheduled based on your availability, from Sunday to Saturday, 8:00 AM – 7:00 PM.
Competitive Pay – Earn $90–$115 per assessment, with potential earnings of $1,800 – $11,500 per month based on the number of visits completed.
Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.
Education and Support – Comprehensive onboarding, a preceptor, 1:1 feedback, and free access to continuing education.
Licensure Support – Assistance obtaining additional state licensure and credentialing in neighboring states if desired.
Transition to Full‑time – PRN providers may transition to full‑time roles with a sign‑on bonus.
Qualifications
Master’s degree or equivalent experience and satisfactory completion of NP licensure.
Current NP licensure in the state of practice, including prescription authority or the ability to obtain it.
Board certification by AANP, AACN, or ANCC in relevant specialties (Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care).
Current BLS, ACLS, or CPR certification.
Preferred: 1 year of NP experience; new graduates encouraged to apply.
Valid state driver’s license and proof of adequate automobile insurance coverage.
Strong computer skills and familiarity with employee health/medical record software.
Excellent verbal and written communication skills.
