Personal Touch Home Care provided pay range: $50.00/hr - $125.00/hr. Your actual pay will be based on your skills and experience.

Nassau, NY. In order to be considered for this job, you must have prior experience working in

Pediatrics

and a valid Registered Nurse (RN) license in the state of NY.

Pay per visit: $50 per visit—$125 per visit. SOC: $100 per visit with an additional $25 bonus if notes are completed within 24hrs. Follow-up/Revisit: $50 per visit with the same bonus.

Job Summary:

The Pediatric Registered Nurse plans, organizes, and directs home care services and is experienced in pediatric nursing from birth through late teens, with an emphasis on community health education and experience.

Responsibilities:

Completes comprehensive assessments of patients and families to determine home care needs and provides a physical assessment and history of current and previous illnesses.

Regularly re‑evaluates patient nursing needs.

Initiates the plan of care and makes necessary revisions as patient status and needs change.

Develops the plan of care with the patient’s physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions, including the patient and family in the planning process.

Initiates appropriate preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed by the physician.

Counsels the patient and family to meet nursing and related needs.

Provides health‑care instructions to the patient/family as appropriate per assessment and plan.

Identifies discharge planning needs and implements them prior to discharge.

Applies high‑tech nursing skills (e.g., catheter insertion/removal, nephrostomy care, ostomy care, infusion services, G‑tube care, negative‑pressure wound therapy).

Maintains infection control procedures and safety precautions.

Shows appropriate application of nursing theory, practice and procedure.

Observes ethical and legal guidelines for safeguarding patient confidentiality.

Assesses patient needs and physical status each visit and re‑evaluates via physical assessment, response to therapy, and updated physician orders.

Communicates changes in patient condition to the physician and obtains/receives orders for plan revisions.

Prepares clinical notes and updates the primary physician, completing documentation per policy.

Coordinates care with physicians and other health‑related persons.

Participates in on‑call duties as defined.

Ensures arrangements for equipment and supplies are available.

Instructs, supervises and evaluates home health aide care every 14 days and LPN/LVN care at least every 30 days.

Exercises professional management and coordinates care across providers.

Qualifications:

Bachelor’s degree in Nursing from an accredited college/university.

Current active and unrestricted nursing license.

Pediatric care experience in the acute and/or home care environment.

Valid driver’s license.

Current certification in Basic Life Support for Health Care Professionals.

Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting.

Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills, and nursing skills per competency checklist.

Basic knowledge of computer operating systems and software applications to communicate and document care.
