SSC‑Sutter Senior Care provides biopsychosocial assessment, crisis intervention, short term counseling, advocacy, and linkage to resources and planning for transitions of care for patients and their families/significant others of all ages in any patient care setting. The role includes psycho‑education, support group facilitation, interdisciplinary consultation, liaison to community programs, discharge planning, staff education, and committee participation.
Responsibilities
Conduct biopsychosocial assessment and provide crisis intervention.
Offer short‑term counseling, grief counseling, and life review/end‑of‑life support.
Facilitate support groups and provide psycho‑education to patients and families.
Consult on psychosocial aspects of care as a member of the interdisciplinary team.
Liaise with community programs and collaborate on discharge planning.
Educate staff and hospital departments and participate on committees as requested.
Qualifications
Master’s degree in Social Work.
Minimum 2 years recent relevant experience.
Clinical skills: biopsychosocial assessment, crisis intervention, family behavioral issues, grief counseling, supportive counseling, adjustment to illness, behavioral change therapy, motivational interviewing, short‑term family counseling, group facilitation.
Collaboration skills and ability to work effectively in a team.
Advocacy skills for patients and families.
Knowledge of transitions of care, community resources, and post‑acute levels of care.
Knowledge of child, elder, and dependent adult abuse, reporting requirements, and regulations (Tarasoff, patient confidentiality).
Assessment and plan of care for suicidal behavior, substance use, and high‑risk behaviors.
Understanding of cultural and spiritual values in practice and age‑specific psychosocial care.
Proficient in computer use, data entry, email, calendar appointments, and task lists.
Knowledge of local resources for at‑risk populations.
Schedule & Hours Full Time, 40 hours per week, 8‑hour days on Monday, Tuesday, Wednesday, Thursday, Friday. Weekend requirements: None.
Benefits Yes. Comprehensive benefits package and competitive pay range of $46.20 to $62.37 per hour.
