Position Summary:
The Licensed Social Worker (LSW) is responsible for supporting the psychosocial well‐being of residents in the assisted living community. This includes conducting assessments, facilitating care planning, advocating for residents’ needs, and providing emotional support to residents and their families. The LSW works as part of an interdisciplinary team to ensure residents receive comprehensive, person‑centered care.
Key Responsibilities
Conduct psychosocial assessments of new residents upon admission and periodically thereafter.
Assist residents and families with adjustment to the assisted living environment.
Develop and implement individualized care plans in coordination with the interdisciplinary team.
Provide counseling and crisis intervention to residents as needed.
Support residents in navigating mental health, financial, legal, and long‑term care services.
Advocate for resident rights and ensure ethical treatment and quality of care.
Facilitate family meetings, care conferences, and support groups.
Monitor changes in residents’ mental and emotional status and report to care team.
Collaborate with external healthcare providers, case managers, and community resources.
Ensure compliance with federal, state, and facility regulations and standards.
Maintain accurate, timely, and confidential documentation.
Qualifications
Bachelor’s or Master’s Degree in Social Work from an accredited program (Master’s preferred).
Current and active state licensure as an LSW, LCSW, or equivalent.
Minimum of 1–2 years’ experience in a healthcare, senior care, or assisted living setting preferred.
Knowledge of geriatric care issues, dementia, and elder abuse reporting requirements.
Strong interpersonal and communication skills.
Ability to handle sensitive situations with empathy and professionalism.
Familiarity with electronic health records and case management systems.
Work Environment
Assisted Living Facility setting.
May require occasional evenings or weekends depending on resident/family needs.
Involves regular interaction with residents, families, staff, and external providers.
