Asante is seeking a
compassionate and dedicated Nurse Practitioner
to join our
Outpatient Palliative Care Team . Based in the heart of
Southern Oregon , this position offers the unique opportunity to provide
holistic, patient-centered care
to individuals with serious or life-limiting illnesses
in the comfort of their homes .

What You ll Do:
Deliver
comprehensive palliative care
in an outpatient and home-based setting. Perform
thorough patient assessments , develop and implement
individualized care plans , and provide symptom management. Prescribe medications , order and interpret diagnostic tests, and provide counseling and education to patients and families. Collaborate closely with a
multidisciplinary team , including physicians, nurses, social workers, chaplains, and other healthcare professionals. Support patients
physical, emotional, spiritual, and social needs
during some of the most critical times in their lives. Participate in
quality improvement, education, and research
