Internal Medicine Nurse Practitioner

We are seeking a nurse practitioner (NP) or physician assistant (PA) for primary care rounding in a behavioral health facility in the Holyoke, MA area. This is a full-time role with no on-call requirements.
Schedule:
4/5 Days per week 8am - 4pm No Call Some weekend and holiday coverage required Responsibilities: Perform primary care in a specialty setting
Manage chronic disease, diagnosis illness and medical conditions, and implement plan of care
Order and interpret lab results
Prescribe and monitor medications
Consult with physicians and coordinate patient care with other healthcare practitioners
Educate patients and their caregivers on coping skills
Build relationships with your patients, patients family members, and facility staff to facilitate effective treatment delivery
Approximately 15 consultations per day Requirements: Primary care experience required
New grads encouraged to apply
Active and unrestricted MA NP or PA license or application in process
Active and unrestricted DEA or application in process
Excellent communication skills
