Job Summary Position Title:
Vice President, Care Management Partnerships
Position Overview:
The Vice President, Care Management Partnerships will lead the development and management of Essen Health Care's Care Management Agency partnership strategy across New York. This executive role drives growth through strategic partnerships with primary care practices while ensuring operational excellence and financial performance. The ideal candidate is a dynamic healthcare leader who combines business development expertise with deep operational knowledge to expand our care management footprint and deliver value to our partner network
Responsibilities Partnership Development
Identify and establish strategic partnerships with primary care practices across New York to expand our care management network
Negotiate and execute partnership agreements that align with organizational goals and ensure mutual benefit
Build and maintain strong, long-term relationships with partner practices through regular engagement and support
Develop partner recruitment strategies and maintain a robust growth pipeline to achieve expansion targets
Operational Management
Oversee care management operations across the entire partner network, ensuring consistent service delivery and quality standards
Ensure regulatory compliance and adherence to quality standards across all partnership operations
Develop scalable processes for partner onboarding, training, and ongoing support
Monitor operational performance metrics and implement continuous improvement initiatives
Financial Oversight
Achieve revenue and profitability targets for the care management partnership portfolio
Monitor financial performance across all partnerships and identify opportunities for optimization
Develop and implement pricing strategies and cost management approaches
Provide regular business performance reporting to executive leadership and stakeholders
Team Leadership
Build, develop, and lead a high-performing cross‑functional team
Foster a culture of excellence, accountability, and continuous improvement
Collaborate effectively with internal stakeholders across clinical, finance, compliance, and other departments
Qualifications Required Qualifications
7+ years of progressive healthcare operations experience in care management or value-based care environments
Proven track record in business development and partnership management within the healthcare industry
Strong understanding of NYS Medicaid managed care and care management regulations
Demonstrated P&L management experience with accountability for financial performance
Excellent negotiation, communication, and relationship management skills
Master's degree in Business Administration (MBA) strongly preferred
Preferred Qualifications:
Deep knowledge of the NYC/Bronx healthcare market and provider landscape
Experience structuring and managing healthcare management services agreements
Background in successfully scaling multi-site healthcare operations
Reports to: Senior Vice President, Care Management & Chief Administrative Officer
