UC San Diego Health is seeking a dynamic and Emergency Department (ED) experienced Registered Nurse (RN) to Manage our innovative ED to Home program. This position is housed within UC San Diego Health Population Health Services and reports to Assistant Director of UCSD at Home. This role oversees an RN CM, Medical Assistant Supervisor and a team of Medical Assistants who follow patients who can transition out of the ED sooner if they meet criteria and agree to enroll in our ED to Home Program. The program functions as a virtual clinic, delivering Tele-Video Visits (TVV) to patient in their homes to promote safe transitions and tight medical management for up to 5 days post ED discharge. In some instances, the ED to Home program may enroll patients who have had a short inpatient stay but need tighter medical management to ensure unnecessary, avoidable ED returns. This person works very closely with our Care Connections Hub and UCSD at Home leaders for tight handoffs and referral between programs, along with our Accent Care Home Health Agency and other community resources. ED to Home has ED MD performing TVV Sunday – Saturday. There may be weekend needs but schedule make accommodations when this need arises. The ideal candidate is committed and passionate to helping build this innovative program with the help of leadership and the Ed to Home team so patients can successfully stay safe at home. Plan is to build in more staff in CY26
Key Responsibilities
Use relational skills to help ED team identify, engage and enroll patients in ED to Home using outlined criteria.
Ability to communicate escalations and day to day priorities to key stakeholders.
Lead and support a team of clinical professionals.
Ensure high-quality, patient-centered care delivery in home settings.
Manage productivity, accountability, and clinical outcomes.
Analyze data to drive performance and continuous improvement.
Collaborate across departments to build a high-performing program.
Ideal Candidate Profile
RN with strong clinical background and leadership experience.
Prior experience in case management or care coordination, and seen as a go-to resource with leadership qualities.
Proficient in EPIC and data analysis tools.

MINIMUM QUALIFICATIONS
A Bachelor's or Master's Degree in Nursing.
Registered Nurse (RN) license issued by the State of California.
A minimum of five (5+) or more years of relevant experience.
Previous RN transfer center, ED RN CM or ED RN experience.
Strong knowledge of discharge planning, transitions of care and clinical disease trajectory.
Demonstrated ability to lead process improvement projects across departments, with strong communication and data skills. Ability to create standard work documents and program policies.
Strong written and verbal communications skills, with the ability to train and mentor subordinate staff, convey complex clinical and technical information in a clear and concise manner, and prepare and present a variety of reports, documentation, analyses, and project proposals.
PREFERRED QUALIFICATIONS
Knowledge of Epic and UC San Diego Health.
Two (2+) or more years of supervisory or management experience.
Experience supervising union-represented employees.
SPECIAL CONDITIONS
Must be able to work various hours and locations based on business needs.
Employment is subject to a criminal background check and pre-employment physical.
Annual Full Pay Range: $132,600 - $259,800 (will be prorated if the appointment percentage is less than 100%)
Hourly Equivalent: $63.51 - $124.43
