Eliot is seeking a
Lead Care Manager
to join our Behavioral Health Community Partner (BH CP) program. This innovative Care Management program serves members with complex medical, behavioral health, and social needs. In this role, you will oversee and supervise a dedicated team of Care Managers. Your leadership will be instrumental in fostering a collaborative environment where effective treatment and program goals are prioritized. The ideal candidate should possess strong communication skills, training expertise, and have a strong commitment to mental health recovery.
Responsibilities:
Responsible for supervision of BH CP Care Manager staff focused on best practices and strategies to meet identified paperwork targets, quality metrics goals, and ACO/MCO initiatives.
Responsible for providing assigned BH CP staff with orientation, training and coaching of BH CP policies and procedures.
Responsible for being BH CP liaison to community resources including assisting and supporting BH CP staff with the connection pathways for communicating with internal agency and external collateral contacts.
Assist and support BH CP staff with connecting Members to Social Determinants of Health (SDoH) resources as indicated on screening tools, Care Plan and/or as indicated by Member needs.
Provide coverage for BH CP vacancies as assigned by a supervisor to ensure timely and effective Members service delivery.
Engage each enrollee in their treatment planning process and their preferences, collaborated with the enrollee’s PCP, and share with the enrollee’s Care Team.
Assist the Care Team and Members by providing on‑going assessment of symptoms and responses to Care Team interventions.
Qualifications:
Bachelor’s degree in psychology, social work or related field
At least two years supervisory experience
Ability to establish supportive relationships with persons with severe and persistent mental illnesses, substance abuse disorders, and complex medical conditions.
Valid driver’s license and a reliable vehicle
Schedule:
Monday‑Friday, 9am-5pm
$65,000 - $65,000 USD
