About the Role: As a Nurse Practitioner in post-acute care, you will provide in-person care to patients in skilled nursing and rehabilitation facilities. In this role, you will partner with facility teams and Trumen-affiliated physicians to manage chronic and acute conditions, support transitional care planning, and contribute to quality-improvement initiatives within your designed facilities.
