Program Manager (RN), Stroke & Sepsis – Palmdale, CA
The Manager for the Stroke & Sepsis Programs will lead, direct, and be accountable for assigned areas. The Program Manager has direct accountability for serving the Stroke & Sepsis programs, with a focus on improving the patient's pathway to stroke & sepsis treatment. This position works closely with physician partners, facility leaders, and the patient care team to manage stroke & sepsis patients from door to discharge, providing education, ensuring the most appropriate discharge disposition, providing coordination of care, and providing follow-up to patients for 90 days post discharge.
The program manager assists with the business planning, marketing, and development for the program, including integration and optimization of patient care. Facilitates daily program operations; demonstrates a thorough understanding of the financial system for budget management, facility policies and procedures, clinical information systems, and business/marketing program management. Role models the facility's mission, vision, and values; supports the learning of others; and practices leader standard work.
Collaborates with Clinical Leaders on program-specific clinical processes to ensure efficiency.
The Program Manager mentors the teams to improve problem-solving, accountability, quality, and satisfaction. Other duties as assigned.
Requirements:
Minimum 3 years of experience as a Registered Nurse in an acute care setting
Experience in Stroke & Sepsis Program Coordination: planning, implementing, and evaluating outcomes
Bachelor's Degree required
Master's Degree preferred
Current license to practice professional nursing in the state of California
Current basic life support (BLS) certification
Advanced National Certification preferred
Salary range - $125,000 - $175,000 (Based on experience)
