Job Summary
Deliver optimal medication management across all dimensions of care.
Ensure the pharmacy department facilitates safe medication use and complies with all legal, accreditation, and certification requirements.
Provide services to patients, nurses, physicians, and other medical staff by interpreting and evaluating the validity and safety of orders and prescriptions.
Perform core clinical activities including order entry, pharmacokinetics, IV to PO therapy, therapeutic interchange, and other required tasks.
Participate in multidisciplinary rounds and direct patient care, contributing to clinical initiatives.
Educate technicians, pharmacists, and other health care practitioners; serve as a preceptor to interns, technicians, and ASHP‑accredited pharmacy residents.
Minimum Qualifications
Bachelor’s Degree in Pharmacy or Pharm. D. from an accredited school of Pharmacy.
License & Certifications
Texas Pharmacist License or eligibility for license; must obtain licensure within 90 days of employment.
Texas Pharmacist Preceptor Certification within 180 days of employment.
ACPE IV Sterile Certification.
American Heart Association Basic Life Support (BLS) certification within 90 days of employment.
Advanced Cardiac Life Support (ACLS) accredited by the American Heart Association (AHA) or LRC; required for inpatient positions prior to or within 6 months of employment.
Board of Pharmacy Specialties Certification preferred; if certified, maintain current status and rectify lapsed certification within 6 months.
Work Experience
Two (2) years of progressive, relevant experience in an OP/Retail setting (for ACSC positions; no residency).
Two (2) years of progressive, relevant experience in an inpatient hospital setting (for IP positions; no residency).
Completion of a PGY1 ASHP‑accredited residency is highly preferred.
Communication Skills
Proficiency in MS Word and basic computer skills.
Knowledge of analytical, design, mathematics, medical terms, research, and statistics.
Work schedule includes weekends, holidays, flexible hours, overtime, travel, and on‑call duties.
