General Summary Under general supervision of the Manager of Case Management, the Case Manager provides care coordination and discharge planning for an assigned patient case load. The Case Manager assesses individual patient needs, coordinates and implements plans of care and monitors and evaluates outcomes across the continuum of care.

Principal Responsibilities and Tasks

Assess assigned patients and integrate psychosocial assessment within the multidisciplinary team.

Develop and coordinate implementation of Discharge Plan A and alternative Plan B, including documentation in the medical record.

Collaborate with physicians, nursing, social work and other disciplines to eliminate barriers to efficient delivery of care.

Facilitate timely transition plans and actively lead inpatient length of stay management and readmission reduction.

Integrate work with ambulatory and clinic transitional case managers and lead care management rounds.

Implement corrective action plans for complex issues and elevate to leads as necessary.

Ensure authorizations are completed timely and collaborate with Utilization Management.

Guide physicians in appropriate clinical documentation and participate in clinical performance improvement activities.

Build a network of positive working relationships that advocate for the patient and remain current on clinical practice protocols.

Participate in department improvement initiatives, orient and educate Case Manager 1s, provide oversight and education to case management interns, and engage in professional growth activities such as certification, degree completion, formal teaching, board membership and peer review.

Lead and promote best practice in day-to-day operations and serve as a resource expert, providing formal training, education or consultation to other departments.

Qualifications

Licensure as a Registered Nurse or another equivalent health care license in the state of Maryland.

Must hold a BSN or equivalent health care degree.

A minimum of 2 years of experience as a case manager.

Current and active specialty certification preferred.

Knowledge of managed care, discharge planning and case management.

Highly effective verbal and written communication skills.

Strong interpersonal and analytical skills with ability to prioritize and work independently.

Ability to work collaboratively with other disciplines and with Hospital/Case Management related software.

Demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served.

Patient Safety

Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.

Takes action to correct observed risks to patient safety.

Reports adverse events and near misses to appropriate management authority.

Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.

Compensation Pay Range: $36.83‑$55.29 per hour.
