The Physical Therapist provides competent, courteous, and respectful evaluation and treatment of patients for difficulties with mobility, function, strength, and positioning per APT scope of practice and licensure laws. The PT communicates and collaborates with families, physicians, and other team members and maintains impeccable professional documentation. The PT also conducts in-services and training for facility staff on restorative and functional maintenance programs. The PT reports to the Therapy Program Manager and ultimately to the Director of Operations. Successful candidates demonstrate the CRI values of Commitment, Respect, and Integrity. Responsibilities
Evaluate patients for rehabilitation services within 24 to 48 hours of physician’s orders. Develop effective treatment plans. Treat patients according to the plan of care. Supervise the licensed Physical Therapy Assistant. Communicate with physicians, clinical managers, and other team members regarding progress, problems, and plans. Participate in Plan of Care Meetings and Interdisciplinary Team Meetings. Achieve or exceed the minimum productivity standard set by CRI. Participate in in-service training programs for other staff. Record treatment notes, progress notes, provide objective input for discharge summaries, and complete daily billing per policy. Instruct patient families and/or facility staff in maintenance programs. Participate in discharge planning. Recommend necessary equipment to appropriate personnel. Report equipment problems to facility personnel. Act as a clinical supervisor for affiliating PT and PTA programs for area universities/colleges. Consistently portray the mission, vision, core values, and professional image of Concept Rehab. Perform special projects and other duties as assigned. Benefits
