Position Overview
Registered Dietitian (RD) opportunity at The Grand Rehabilitation & Nursing at Pawling. Support resident nutrition, wellness, and clinical outcomes in long‑term care.
Location: The Grand Rehabilitation & Nursing at Pawling, Duchess County, NY. Pay: $65,000.00 to $78,000.00 with benefits; negotiable. Position Type: Full‑Time.
Key Responsibilities
Conduct nutritional assessments and develop individualized care plans
Monitor intake, weight trends, and clinical outcomes
Modify dietary plans based on medical conditions and resident needs
Develop and oversee therapeutic and balanced meal plans
Ensure diets align with physician orders and resident preferences
Maintain consistency across all dietary programs
Educate residents, families, and staff on nutrition and wellness
Collaborate with nursing, providers, and interdisciplinary teams
Provide guidance and training to dietary and kitchen staff
Ensure compliance with state and federal regulations
Maintain accurate nutrition‑related documentation
Support survey readiness and quality improvement initiatives
Qualifications
Bachelor’s degree in Dietetics, Nutrition, or related field
Current Registered Dietitian (RD) credential required
Minimum 2 years of dietitian experience (long‑term care or healthcare preferred)
Strong communication and organizational skills
Ability to work independently and as part of an interdisciplinary team
