Pay: $30.00 / hr. – $45.00 / hr. (Depending on experience and responsibilities)

Responsibilities
Obtain a medical history and assess the patient’s physical and emotional status, level of competency, home environment, safety factors, and family/household member’s ability to assist with care.
Advocate for the patient by providing information on patient rights, grievance procedures, reimbursement procedures, and patient/family responsibilities; involve family in developing the plan of care.
Participate in updating the interdisciplinary care plan every two weeks, including implementing the correct interventions to attain goals.
Supervise and direct the care given by a Hospice Aide/CNA and evaluate such care in accordance with guidelines and regulations.
Observe signs, symptoms, and reactions to treatments and inform the attending physician and/or PCC immediately of any changes.
Participate in Performance Improvement activities.
Participate in on‑call rotating schedule and provide back‑up assistance as requested.
Accurately document in the electronic medical record using any required automated device.
Qualifications
Must be a Registered Nurse licensed in the state of operation.
Experience required, with experience in Hospice or Home Health preferred.
Sound knowledge of nursing principles, procedures, and elements of patient family teaching.
