Job Overview The Wellness Director reports to the Executive Director and coordinates nursing services for residents, ensuring compliance with state regulations and quality of care.

Responsibilities

Ensure clinical services comply with state regulations and community policies.

Coordinate nursing and personal care services for residents.

Provide nursing services including resident instruction, wellness counseling, health promotion, disease prevention, and chronic condition care.

Verify medication regimens with physicians for residents requiring supervision.

Assist residents with activities of daily living as needed.

Coordinate wellness reviews, care plans, and move‑in/out processes.

Collect and review resident health data, identify assessment needs, and initiate well‑ness plan reviews.

Coordinate and participate in staff training, orientation, ongoing education, and supervision.

Maintain accurate documentation, delegation, and compliance with RN responsibilities per state regulations.

Provide after‑hours support for resident needs and emergency procedures.

Manage infection control, immunizations, and disease management.

Assist with and discontinue services and conduct quality assurance audits.

Create monthly staffing schedule and weekly reports to the General Manager.

Participate in community‑related duties and communicate effectively with staff and leadership.

Qualifications

Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN) licensed in the state.

Either a baccalaureate in nursing with at least 2 years of full‑time clinical experience (1 year in home healthcare or community health program) or an associate/diploma degree with at least 4 years of full‑time experience (1 year in home healthcare or community health program). Experience must be within the last 10 years.

Experience and passion for working with seniors.

Physical ability to lift/push/bend up to 60 pounds, perform frequent lifting, pushing, pulling, moving equipment, bending, stooping, and remain on feet for long periods.

Strong interpersonal skills to maintain positive relationships with residents, families, peers, and staff.

Good physical and emotional health, free of communicable diseases.

Ability to react quickly in any situation.
