Primary Responsibilities

Evaluate the patient’s functional status and physical therapy needs, and assist the physician in developing the therapy plan of care.

Provide services within the scope of practice defined by state laws governing physical therapy, in accordance with the plan of care, and in coordination with other health‑care team members.

Observe and report the patient’s response to treatment and any changes in his or her condition to the supervising nurse and/or the physician.

Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.

Instruct and inform patients, family/caregivers, and the health‑care team regarding the plan of care, functional limitations, and progress toward goals.

Visit patients according to the plan of treatment, complete a progress note for each visit, and submit progress notes per LHC policy.

Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines.

Report outcomes of evaluation, goals, and anticipated projected frequency of care.

Required Qualifications

Current Physical Therapy licensure in the state of practice.

Current CPR certification.

Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Pay and Benefits: The hourly pay for this role ranges from $34.23 to $61.15 per hour based on full‑time employment. We offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits are subject to eligibility requirements). We comply with all minimum wage laws as applicable.
