Overview ERP International is seeking a full‑time Registered Nurse – Case Management in support of the 56th Medical Group at Luke AFB, AZ. Responsibilities Work schedule: Monday–Friday, 9‑hour days between 7:00 am and 5:30 pm with a one‑hour lunch. No weekends or holidays.

Positon Summary: Provides case management, care coordination, and discharge/disposition planning for inpatient and outpatient care settings. Assists service members, veterans, family members, and caregivers in accessing appropriate services for complex health‑care needs, including acute, chronic, catastrophic, or life‑threatening illnesses, combat stress, traumatic brain injury, community adjustment, addictions, and other health problems. Coordinates care across providers, addresses psychosocial and medical needs, and evaluates outcomes to ensure timely, appropriate services.

Assessment: Identify and evaluate patients and families for case management from multiple sources, conduct systematic collection of patient status via direct contact and records.

Planning: Develop patient‑specific care plans with short‑ and long‑term goals, coordinate and obtain approval from patient, family, providers, and team.

Implementation: Guide patients and families through the health‑care system, activate plans, secure referrals, advocate, and provide education.

Coordination: Ensure smooth care delivery across settings, maintain relationships with military and community resources, and optimize access to services.

Monitoring: Document and update treatment plans per DoD, AF, and agency guidelines; maintain records and data collection; participate in program evaluation.

Evaluation: Monitor adherence, response, timeliness, identify variances, measure outcomes, and evaluate the overall case‑management program.

Core Duties

Participate in all phases of the Case Management Program and ensure adherence to CM standards of care.

Provide nursing expertise for assessment, planning, implementation, coordination, and monitoring.

Develop and implement local CM strategies using inpatient, outpatient, onsite, and telephonic care.

Create tools to support case management, including identification, assessment, guidelines, algorithms, CM software, and databases.

Integrate CM and utilization management, including nursing and social‑work collaboration.

Maintain liaison with community agencies and accurately document patient care data.

Develop treatment plans and ensure continuity of care for optimal wellness.

Ensure proper implementation of patient treatment plans and follow‑up post‑discharge.

Provide health‑care instruction to patients and caregivers based on learning needs.

Qualifications Minimum Education:

Minimum ASN; BSN preferred for case management.

Experience:

Minimum 3 years for ADN or 2 years for BSN in full‑time clinical case management with adults, children, families, seniors, and groups.

Licensure:

Current, full, active, unrestricted Registered Nurse license to practice in any state.

Clinical Certification:

Certified Case Manager by CCM or ANCC, or certification eligible with 3 years of experience for ADN or 2 years for BSN.

Life Support Certification:

Current AHA or ARC BLS Healthcare Provider certification.

Security:

Ability to pass a Government background check and obtain a security clearance.

Additional Required Skills and Knowledge

Core case‑management functions: assessment, planning, facilitation, advocacy.

Knowledge of HIPAA, accreditation standards (AAAHC, TJC), and computer applications (Microsoft Office, Outlook, internet).

Interpersonal skills for communicating with patients, families, and staff, including those who are physically or mentally ill, uncooperative, or emotionally distressed.

Organizational, problem‑solving, and communication skills for articulating medical requirements to patients and staff.
