Licensed Practical Nurse / Registered Nurse SUMMARY:

In need of an LPN or an RN who will provide direct nursing care to residents and supervise the day‑to‑day nursing activities performed by nursing assistants. Supervision must follow current federal, state, and local standards, guidelines, and regulations that govern our facility, and it may be required by the Director of Nursing to ensure the highest degree of quality care is maintained at all times.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Direct the day‑to‑day functions of the nursing assistants. Meet with assigned nursing staff and support personnel to plan the shift’s services, programs, and activities. Provide written and oral reports/recommendations concerning the activities of the shift as required. Admit, transfer, and discharge residents as needed. Provide leadership to nursing personnel assigned to your unit/shift.

Complete and file required record‑keeping forms and charts upon the resident’s admission, transfer, or discharge. Receive and transcribe telephone orders from physicians and record on the Physician’s Order Form. Chart nurse notes in an informative and descriptive manner that reflects the care provided and the resident’s response. Fill out and complete accident/incident reports and submit to the Director as required. Perform routine charting duties in accordance with established policies and procedures.

Prepare and administer medications as ordered by the physician.

Develop and maintain good working rapport with inter‑departmental personnel and other departments within the facility to ensure nursing services and activities can be adequately maintained to meet resident needs. Create and maintain a warm, positive, and calm environment throughout the unit and shift. Receive and give the nursing report upon reporting in and ending shift duty. Make rounds with physicians as necessary.

Administer professional services such as catheterization, tube feedings, suction, dressing changes, colostomy care, blood sampling, massages, and range‑of‑motion exercises, as required.

Monitor assigned personnel to ensure adherence to established safety regulations in the use of equipment and supplies.

Maintain confidentiality of all resident care information, including protected health information. Report known or suspected incidents of unauthorized disclosure. Ensure all nursing care is provided in privacy and that personnel knock before entering the resident’s room.

Provide IV and IV site care as needed.

Perform other duties as assigned.

QUALIFICATIONS

Registered Nurse with current unencumbered state licensure.

Licensed Practical Nurse with current unencumbered state licensure.

Must be able to read, write, speak, and understand English.

Must be able to make independent decisions when circumstances warrant such action.

Must be knowledgeable of nursing and medical practices, procedures, laws, regulations, and guidelines that pertain to nursing care facilities.

Current CPR certification is required.

Experience in long‑term care is preferred.

PHYSICAL DEMANDS

Be able to move intermittently throughout the workday.

Be able to cope with the mental and emotional stress of the position.

Meet general health requirements, including a medical and physical examination.

Push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and carry such weight a minimum distance of 50 feet.

Assist in the evacuation of residents during emergency situations if necessary.
