Home Health Physical Therapist (PT) – Elgin, IL
Safe Life Home Health Care, LLC was founded in 2010. We are a professional, customer‑centric, fast‑paced home health agency. We maintain state licensure, CHAP accreditation, and Medicare certification. We serve 13 counties in Illinois and provide multidisciplinary care.
Pay Base pay: $80,000.00/yr – $100,000.00/yr (or $45–$50 per hour for part‑time).
Location Elgin – Illinois (covering 13 counties in Illinois).
Position Overview Provides Physical Therapy services to patients as directed by plan of care in line with the philosophy, objectives, and policies of the agency. Supervises Physical Therapy Assistants and maintains responsibility for the implementation of follow‑through of PT procedures.
Responsibilities
Assists the physician in evaluating patients’ level of function, establishing treatment plan and goals.
Performs initial evaluations within 48 hours of referral and re‑evaluations every 30 days, as appropriate, coordinating plan of care with the physician as needed.
Prepares clinical and progress notes and required summaries, as appropriate.
Revises plan of treatment and contacts the physician for change in plan of care.
Advises and consults with patient’s family and other health‑care staff.
Coordinates care with agency DON and other staff as appropriate.
Observes, records, and reports any changes in the patient’s condition and updates the agency on changes in the patient’s status or schedule changes.
Supervises the Physical Therapy Assistant.
Instructs patient and/or family in the use of equipment, safe patient management, and home therapy programs as needed.
Communicates effectively with patients, caregivers, and families as well as agency staff.
Provides quality and appropriate care and reports issues or concerns to the DON/Administrator.
Ensures that all tasks are performed in a timely manner.
Checks in on Physical Therapy Assistants every 4–6 patient visits.
Qualifications
Licensed Physical Therapist in the state of Illinois.
At least one year of Home Health Care clinical experience.
Valid driver's license with proof of insurance.
Maintains annual TB test, CPR certification and other required certifications.
Ability to read and interpret instructions related to patient care.
Ability to effectively perform and prioritize multiple functions or tasks as needed.
Must be able to travel to prospective patient’s place of residence.
