Join Citadel Healthcare as a Licensed Practical Nurse (LPN) and make a real difference in the lives of our residents. The LPN will assume responsibility for a group of residents during a shift of duty, providing quality nursing care and collaborating with the inter‑disciplinary team.

Base Pay Range $34.00/hr – $45.00/hr

Responsibilities

Provide licensed care to assigned residents as ordered by the physician and in accordance with facility, federal, state, and local standards, guidelines, and regulations.

Receive and provide nursing reports upon reporting in and ending shift duty hours.

Maintain a clean and sanitary work area, including nurse station, medication cart, medication room, and treatment areas.

Direct and supervise the day‑to‑day Certified Nursing Assistant services for the assigned unit to ensure care is rendered according to current standards.

Conduct required resident assessments, observations, and interventions, reviewing care plans to ensure appropriate care.

Take and record residents' blood pressure and vital signs; administer catheter care, IV therapy, and respiratory care (oxygen, nebulizer treatments) as ordered within license limitations.

Transcribe physician's orders into resident charts, MAR, treatment plans as required.

Prepare and administer medications and perform treatments as ordered by the physician.

Chart nurse's notes in a descriptive manner reflecting the care provided and the resident's response.

Complete required forms upon admission, transfer, and/or discharge.

Ensure the resident's environment is free of accidents and hazards; promptly address potential safety risks.

Make written and oral reports/recommendations concerning shift activities as required.

Implement and maintain all aspects of resident care plans; recommend changes per professional standards.

Perform any other duties as assigned.

Skills & Abilities

Excellent written and verbal communication skills in English.

Ability to multi‑task and remain calm under stress.

Analytical and problem‑solving skills.

Knowledge of infection control standards, policies, and procedures.

Ability to adapt to a flexible work schedule, including occasional overnight travel.

Basic computer proficiency and aptitude to learn company applications.

Education & Experience

Current CPR certification required.

Valid LPN license in the state.

Preferred: 1 year of experience in long‑term care.

Physical Requirements

Walking, reaching, bending, lifting, extended sitting, fine hand coordination, pushing, and pulling, with or without mechanical aids.

Ability to handle limited environmental hazards and adhere to company safety standards.
