Albany Medical Health System seeks a Registered Dietitian/Registered Dietitian Nutritionist and Certified Diabetes Care & Education Specialist (RD, CDCES) to join our academic Endocrinology practice at Albany Medical Center. This opportunity is within a multidisciplinary team in a dynamic, patient-centered environment. The RD, CDCES plays a vital role in providing expert guidance, education, and support to individuals with diabetes and related conditions. The professional will assess patients, develop personalized care plans, deliver diabetes self-management education, and collaborate closely with the healthcare team to optimize outcomes.
Key Responsibilities
Conduct thorough assessments of patients’ nutritional status, diabetes knowledge, self-care skills, and emotional well-being. Create and implement personalized diabetes management plans that reflect patient goals, preferences, and cultural considerations. Provide education on blood glucose monitoring, insulin pump therapy, medication adherence, healthy eating, physical activity, and coping strategies to empower patients in their self-care. Offer evidence-based nutrition guidance to support blood glucose control, weight management, and the prevention or management of diabetes-related complications. Work closely with physicians, nurses, pharmacists, and other team members to deliver coordinated, high-quality care. Participate in quality initiatives aimed at enhancing diabetes education and care delivery within the practice. Serve as a resource and advocate for individuals living with diabetes, offering ongoing support and encouragement. Engage in community education and outreach activities to promote diabetes awareness and prevention. Maintain accurate and timely documentation of assessments, care plans, and interventions within the electronic health record (EHR). Qualifications
Registered Dietitian (RD) or Registered Dietitian Nutritionist credential required. Certified Diabetes Care and Education Specialist (CDCES) certification preferred; candidates must be willing to obtain certification within a specified timeframe if not already certified. Strong understanding of diabetes pathophysiology, treatment approaches, and current evidence-based guidelines. Excellent communication, teaching, and counseling skills with a focus on patient engagement and empowerment. Demonstrated ability to work effectively as part of an interdisciplinary team.
