Care Navigator - PCMG

Position Summary The Care Navigator supports care coordination by guiding patients and families through the healthcare system. Working under the direction of a licensed healthcare professional, this role provides administrative and coordination support to ensure patients receive efficient, effective, and customer-focused care.
Key Responsibilities
Coordinate patient care activities and services
Assist patients and families with navigating healthcare resources
Communicate with providers and care teams
Maintain accurate documentation in electronic medical records
Provide high-quality customer service and support
Required Qualifications
High School Diploma or equivalent
Associate’s degree in a related field or equivalent experience
3+ years of experience in healthcare or related field
Knowledge of medical terminology
Strong communication, problem-solving, and critical-thinking skills
Proficiency in Microsoft Word, Excel, and Electronic Medical Records
Position Duties Care Management Support
Verifies insurance eligibility and performs validation checks to ensure accurate contact information.
Verifies demographic information for coordinating communication.
Meets with patients/caregivers during clinic appointments, documents in electronic medical record, coordinates time with the family for completion of patient forms and provides updates to the licensed health care provider.
Executes administrative portions of care coordination under the direction of the licensed health care professional and in cooperation with the health care multidisciplinary team.
Obtains medical records such as outside films, charts, pathology and other historical data as needed.
Additional Activities
Providing written patient education or resources as directed by the licensed health care provider.
Communicating patient/caregiver needs or barriers to the health care multidisciplinary team.
Facilitating phone/fax/email communication to expedite tasks.
Re‑scheduling or coordinating follow‑up appointments to reduce family travel.
Verifying readiness of prescribed medications at outpatient pharmacy.
Communicating surgical readiness and plan with family and health care team.
Logs all encounters with and about patients in the electronic medical record.
Escalates barriers to implementing the care plan to the licensed health care provider and health care team.
Program and Population Support
Manages data collection and entry.
Maintains databases.
Coordinates new patient referrals.
Assists with marketing program services and recruitment.
Assists with recruitment and onboarding of new team members.
Assists with administration of survey tools to patients/families, providers, and care management associates.
Maintains accurate department records.
Identifies and provides input to enhance effectiveness of systems and processes.
Customer Service
Communicates clearly in both written and verbal communications with all internal and external customers.
Provides excellent service routinely in interactions with all internal and external customers.
Collaborates with others to acquire information and materials.
Answers telephone calls, takes messages, maintains schedules.
Responds to email within 24 hours or one business day.
Assists with coordination and scheduling of procedures, clinic visits, and other activities as directed by licensed health care provider.
Education
Participates in a variety of educational programs to maintain current skills and competency levels.
