Facilitate communication among providers, patients, and families for cohesive care.
Participate in clinical research to improve practices and outcomes.
Maintain accurate documentation adhering to HIPAA and policies.
Coordinate resources and services across various settings.
Qualifications Qualifications include a Master’s in Nursing focusing on Palliative Care, active Nurse Practitioner certification, and relevant experience. Proficiency in EMR systems and knowledge of pain management techniques are required. Strong communication skills are essential.
