Home-Based Therapist
We are seeking a compassionate Home-Based Therapist to deliver in-home mental health services to children, adolescents, and families in Bad Axe, MI.
Responsibilities
Provide clinical assessments, develop and monitor individualized family plans, and conduct regular home visits to support parenting and family dynamics.
Maintain consistent contact, document progress, and liaise with community agencies such as schools and DHS.
Offer crisis stabilization and connect families to community resources.
Participate in program planning and community education activities.
Qualifications Master’s degree in Social Work, Counseling, or Psychology; MI licensure (LLMSW or LMSW); minimum 2 years’ experience preferred; valid driver’s license; willingness to complete Infant Mental Health endorsement.
