Job Summary and Qualifications The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The PT completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The PT provides or directs the majority of the patient’s physical therapy to support patients throughout the continuum of care.

Responsibilities

Demonstrate and articulate sound clinical reasoning through synthesizing evaluation data from initial patient assessment, medical history and physician orders to develop a comprehensive plan of care.

Communicate and interact with patients, families and others in a caring, empathetic, courteous, and respectful manner.

Receive, interpret, and implement physician orders accurately and in a courteous manner.

Consult resources to clarify unfamiliar or questionable orders.

Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.

Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.

Provide purposeful and skilled therapeutic care at patient’s home, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.

Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.

Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.

Requirements

Graduate of an accredited school of Physical Therapy.

B.S. or M.S. in Physical Therapy.

State of Florida licensure as a Physical Therapist.

Current certification as a BLS Healthcare Provider (American Heart Association).

Customer service abilities including effective listening skills.

Critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast paced environment.

Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/or equipment up to 50 pounds.

Basic Cardiac Life Support must be obtained within 30 days of employment start date.

Licensed Physical Therapist.

Bachelors Degree.
