Office Location: Lawrenceville, GA Coverage area: Roswell, Marietta, and Cumming Schedule: Monday thru Friday 8am-5pm Role

Assess and monitor physical, emotional, and psychological needs of patients Create hospice care plans aligned with patients’ wishes and goals Administer medications, treatments, and interventions Provide pain and symptom management Educate and support patient’s family and caregivers Collaborate with interdisciplinary team Maintain accurate and timely documentation Participate in on-call rotation as required by local branch Qualifications
Graduate of an accredited nursing school with current licensure to practice in state of operation One year nursing experience required; 3+ years preferred Experience in hospice or a similar setting preferred Knowledge of the hospice philosophy of care Commitment to clinical and documentation excellence Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
