Volunteers of America-Greater New York (VOA-Greater New York) is an anti-poverty organization working to end homelessness in the New York area by 2050. We operate over 60 programs across New York City, Northern New Jersey and Westchester County including emergency, transitional and permanent housing. Our services extend to families experiencing homelessness, survivors of domestic violence, people living with HIV/AIDS, veterans, youth aging out of foster care, adults with behavioral health or substance use needs , and others in need, including a special education preschool program.

Position: Social Worker

The

Social Worker

creates and maintains a wellness and recovery-oriented environment by using person-first language, respectful documentation, person centered planning, and evidence-based practices to ensure the program provides comprehensive and integrated on-site social service that helps residents achieve their goals and successfully age in place. The Social Worker is responsible for the implementation of the treatment program in accordance with policies and procedures, program models, funding source standards, and evidence-based practices. The Social Worker is responsible for meeting the highest possible standards of health and safety and provides all needed quality services to residents to meet contractual and regulatory requirements.

Minimum Qualifications

Masters Degree in Social Work with the expectation that LMSW licensure is secured within 1 year of hire. At least two years of relevant experience.

Position requires a reasonable combination of skills in the following areas: strengths based, solution focused practice.

NOTE: This position is in a qualified setting for clinical experience hours towards LCSW licensure.

Meet with residents to provide treatment, conduct psychosocial assessments, and develop individual service plans from intake through discharge in accordance with the program regulations.

Provide appropriate case management and therapeutic interventions focused on senior services including housing stability, medical and mental health stability, again in place, social isolation, etc.

Maintain case records and statistics for program.

Ensure support and education are provided and documented to assist residents in their transition into the community including referrals to appropriate social service agencies when the facility does not provide the needed service.

Provide ongoing risk assessments, crisis intervention and emergency response to address resident critical incidents.

Develop community linkages and refer residents to appropriate community providers to address their needs.

Work closely with hospitals and emergency rooms to coordinate mental health and medical services for residents.
