The Role Anthos|Home is seeking a compassionate and detail-oriented Aftercare Specialist – Housing Stability to join our Aftercare Team, working on our STEHP Eviction Prevention Program. Reporting to the Aftercare Operations Supervisor, the Specialist will play a critical role in delivering services to at-risk households, fostering positive landlord-tenant relationships, and ensuring compliance with program standards. The Aftercare Specialist – Housing Stability will engage directly with participants, offering tailored support to enhance housing stability and prevent homelessness.

Key Responsibilities Participant Support & Housing Stability (STEHP Program)

Conduct intake assessments to determine eligibility and identify participant needs in alignment with STEHP guidelines.

Develop and implement strengths-based service plans focused on budgeting, employment, and long-term housing stability.

Provide ongoing support through monthly check-ins (in person, by phone, or virtually) to monitor progress and address challenges.

Facilitate tenant education on rights, responsibilities, and housing retention strategies.

Serve as a liaison between participants and landlords, supporting communication and helping resolve issues related to lease compliance, rent arrears, and housing stability.

Coordinate with external service providers to connect participants to mental health, legal, employment, and other supportive resources.

Arrears Support, Documentation & Program Coordination

Review and analyze rent ledgers to ensure accuracy and support determinations for rental assistance and arrears resolution.

Document case activity, participant progress, and arrears-related communications in Salesforce and other required systems in a timely and accurate manner.

Maintain complete, confidential participant files and ensure data quality to support reporting, compliance, and audit readiness, including HMIS requirements.

Prepare and submit payment requests, assist with vendor setup, and coordinate approved payments in accordance with internal procedures.

Team & Operational Support

Support Aftercare Team operations by coordinating and tracking LOSU pickups, exchanges, and citywide deliveries, ensuring proper documentation and timely completion.

Provide flexible caseload support and coverage for Aftercare participants as needed to meet program demands.

Complete administrative and operational tasks that support the effective delivery of Aftercare and STEHP services.

Participate in an on-call rotation to respond to urgent participant needs outside of regular business hours, as needed.

Additional Responsibilities

Carry out other related duties as assigned in support of program goals, team operations, and participant success.

Qualifications and Skills

Bilingual in Spanish/English

Bachelor’s degree in Social Work or a related field, and/or 4-6 years of experience working with people in the housing and human service sector.

Passion for the mission of Anthos|Home and the ability to work in a diverse environment.

Exceptional organizational and record-keeping abilities, coupled with strong problem‑solving skills.

Strong verbal and written communication skills.

Proficiency in MS Word, Excel, PowerPoint, and Salesforce.

Ability to use public transportation or meet travel requirements with reasonable accommodation.

Ability to ascend/descend stairs at locations without elevator access.

Compensation and Benefits

Competitive starting salary: $65,000 - $70,000

Hybrid work environment (3‑5 days in office/field per week)

Tuesday – Saturday schedule a possibility
