Job Responsibilities

Admissions, Discharge and Transfers of residents

Medication requisitions with pharmacy and physicians

Medication administration

Follow infection control, emergencies, and other center policies

Collaborate with resident’s interdisciplinary team to provide the best outcomes

Identify and report all abnormal changes in residents’ condition to proper personnel per center policy

Work under direct supervision in accordance with the state‑specific Nurse Practice Act, facility policies and procedures, and nursing judgment.

Deliver nursing care to patients/residents requiring long‑term or rehabilitative care.

Collect patient/resident data, make observations, and report pertinent information related to the care of the patient/resident.

According to state‑specific regulations, implement the patient/resident plan of care and evaluate the patient/resident response.

In accordance with state‑specific regulations, direct and supervise care provided by other nursing personnel in selected situations.

Maintain knowledge of necessary documentation requirements.

Maintain knowledge of equipment set‑up, maintenance and use, e.g., monitors, infusion devices, drain devices, etc.

Maintain confidentiality and patient/resident rights regarding all patient/resident/peronal information.

Qualifications

LPN (Licensed Practical Nurse) licensed by the State

CPR Certification required

Two years prior nursing experience preferred
