Position Overview The
MDS Coordinator
is responsible for the timely and accurate completion of the RAI/MDS process from admission through discharge. This role works closely with the interdisciplinary team to ensure regulatory compliance, accurate reimbursement, and high‑quality resident care.
Key Responsibilities
Coordinate and manage the RAI/MDS process in compliance with federal and state regulations
Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews
Ensure accurate MDS coding supported by appropriate clinical documentation
Monitor resident status and identify significant changes impacting care plans and assessments
Maintain accurate and timely data entry in PointClickCare and other MDS systems
Submit MDS documentation to the state and manage validation error follow‑up
Participate in care plan meetings, quality assurance activities, and survey readiness
Track discharges, re‑entries, Medicaid, and PPS requirements
Report audit findings and trends to the Quality Assurance Committee
Qualifications
Graduate of an approved LPN or RN program; active state license required
2+ years of skilled nursing experience preferred
Working knowledge of MDS 3.0, RAI process, OBRA regulations, and care planning
Knowledge of Medicare PPS, Case‑Mix, and Medicaid reimbursement
Experience with PointClickCare preferred
Strong organizational, documentation, and communication skills
