Job Summary The Occupational Therapist provides high‑quality, patient‑centered care by managing the complete process of occupational therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team. The therapist completes patient assessments and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and a positive patient experience. The therapist provides or directs a majority of the patient’s physical therapy to support patients throughout home health and the continuum of care.

Responsibilities

Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.

Assess and evaluate therapeutic/rehabilitative/functional status and participate in patient placement and the development of the total plan of care.

Provide purposeful and skill‑based interact‑ion care using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.

Observe and report the patient’s reaction to treatment and changes in the patient’s condition.

Communicate pertinent payer needs/requirements to physician, treatment team, patient and patient’s family in a timely manner.

Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.

Qualifications

Graduate of an American Occupational Therapy Association accredited school with bachelor’s, master’s or doctorate degree in Occupational Therapy.

Maintains current registration with the National Board for Certification in Occupational Therapy.

Maintains active Occupational Therapy Certification in the State of Colorado.

Maintains current certification in BLS for HealthCare Providers (in accordance with American Heart Association guidelines). Basic Cardiac Life Support must be obtained within 30 days of employment start date.

Successful completion of student clinical rotation or work experience in at least one of the following settings: acute care hospital, long‑term acute care facility, skilled nursing facility, and/or acute rehabilitation setting.

(OTR) Occupational Therapist Registered, or Bachelor’s Degree.

No Travel Required.

2 Years Experience Preferred.
