Base pay range $38,760.00/hr - $45,344.00/hr

POSITION SUMMARY The Wraparound Facilitator is responsible for facilitating the provision of family preservation and support services to families eligible for Wraparound services. The Facilitator works in collaboration with the Wraparound Services Team members to accomplish and assist: identifying informal and formal supports, facilitating completing psycho‑social strength assessments, creating a family centered plan, monitoring the implementation of the plan, and ensuring regular (at least weekly) meetings with the Team. Acting as an advocate for the youth and family, they enhance awareness of mental health needs, diversity, strengths, and culture while also providing documentation for all services delivered in accordance with agency standards and guidelines established by funding sources. Services will be provided in the office, the clients’ homes, school, or other appropriate community settings.

Qualifications

Bachelor’s degree in human services or related field.

Valid Michigan Driver’s License, Insurance, and a good driving record.

Must be able to complete the required Michigan Department of Health and Human Services (MDHHS) Services 3-day Wraparound Services training in Lansing, MI.

Must complete 2 MDHHS required Wraparound trainings annually.

A minimum of one (1) year of responsible professional post‑degree mental health experience, specifically with severely emotionally disturbed children preferred but not required.

Understanding of criminal justice delinquency, neglect/abuse issues, and organizational and family systems and/or social work practices as it relates to family systems.

Must tolerate varied weather conditions when traveling to client’s homes, training courses or other community events related to program and agency requirements.
