Bronx Health Collective – Open Clinical Social Worker Position

Hourly Rate:
$39.40.
Organization Overview The Bronx Health Collective (BHC), affiliated with the Montefiore Health System‑Albert Einstein College of Medicine, is a federally qualified community health center that provides comprehensive primary care to over 9,000 children, adolescents, and adults annually. Founded in 1987, BHC delivers integrated mental health care, offering services through shelters, mobile vans, and a clinical site. The current position is part of the Home Opportunities for Mobile Empowerment (HOME) program, focusing on psychotherapy for families experiencing homelessness and housing instability.
Clinical Social Worker Responsibilities
Provide individual, group, and family therapeutic interventions for children, adolescents, adults, and families.
Complete biopsychosocial assessments for new patients.
Write letters of support and evaluations for housing, immigration, and other issues related to client mental health.
Document care in the electronic medical record in a timely manner.
Attend and actively participate in mental health team meetings and events.
Coordinate care with medical providers across an interdisciplinary team.
Serve as a mental health consultant to medical providers during regular work hours as needed.
Qualifications and Requirements
Valid New York State license: LMSW or LCSW.
Minimum of 2 years of clinical experience preferred.
Excellent verbal and written communication skills.
Experience with social determinants of health.
Bilingual in English and Spanish required.
Eligible for the Loan Repayment Program via HRSA (HPSA Score 19).
Salary and benefits are furnished upon request.
