Position Overview
Berkshire Meadows is a residential facility for children and adults with severe intellectual disabilities and complex medical needs. Direct Support Professional will oversee the implementation of a behavior management system and supports. This position includes data monitoring and assisting in the development of treatment plans with other members of a multidisciplinary team.
Essential Job Functions
Professionally support adult or pediatric individuals with an intellectual and/or developmental disability in their living, working, or community settings to promote emotional, physical, and personal well‑being.
Implement strategies to assist the individual in obtaining outcomes stated in the Individual Support Plan (ISP).
Complete documentation to substantiate services delivered (EVV, daily service note).
Assist the individual and the interdisciplinary team in developing and maintaining the ISP.
Immediately report any changes or problems in the individual's condition to the manager.
Complete all documentation in accordance with state and program regulations, Aveanna HealthCare policies and procedures, and accreditation standards.
Always adhere to confidentiality standards and professional boundaries.
Assist individuals in reaching goals to be more independent by offering guidance, instruction, and role modeling.
Actively seek opportunities for individuals to experience life in their homes and communities based on their hobbies and interests.
Support individuals' preferences and ensure their interests are honored.
Assist in development of appropriate life skills necessary to reach highest potential (interaction with others, cooking, housekeeping, budgeting, personal hygiene, etc.).
Assist living an everyday life.
Ensure the health and safety of the individual.
Provide scheduling information: 2nd shift 3 pm‑11 pm and 3rd shift 11 pm‑7 am open everyday; 1st shift 7 am‑3 pm only on weekends.
Respite Only Requirements
All members of household 18 and older must have criminal history checks.
Provide proof of homeowners/renters insurance.
Requirements
The desire to assist individuals to live life to its fullest.
High school diploma or equivalency.
Active driver’s license, current automobile insurance, current car registration and inspection if transporting.
Willingness to accompany those you serve into the community and provide support while maintaining sensitivity to civil and human rights and dignity of the individual.
Must be 18 or older.
Ability to work flexible hours as needed.
Eligibility for State Police Criminal Record Check and Child Abuse History Clearance.
FBI clearance required for individuals who do not meet PA residency requirements.
