Proficiency in HomeCare HomeBase (HCHB), and OASIS experience Responsibilities
Coordinate and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures. Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses. Conduct home visits and attend periodic office meetings. Responsible for managing a caseload of 5-10 patients per day, translating to roughly 6-8 points. Handle Start of Care, Wound Care, Follow-Up Visits, etc. Maintain documentation proficiency and complete all necessary paperwork within 24 hours of the patient appointment. Must be open to on-call rotations and weekend shifts, as required. Required to travel up to 75 miles from the home clinic and have reliable transportation with a valid driver's license and liability insurance. Compensated for mileage between patient homes, except for the commute to and from the first and last patient. Assigned to cover Tucson and surrounding areas, traveling approximately 150-250 miles per week. Attend weekly meetings at the clinic and maintain effective communication with the Clinical Manager. Must accept all patient assignments and maintain productivity expectations of 30 points / week.
