Rates
Licensed Master Social Worker (LMSW): $43 per hour
Licensed Clinical Social Worker (LCSW): $46 per hour
In‑Person Patient Evaluation: $60 per visit (Telehealth: $40 per visit)
Pediatric Specialty: $69 per visit (Telehealth: $40 per visit)
Orientation (classroom): $165.00 per day
Responsibilities
Provide psychosocial support and Social Work services to Hospice patients and their families in inpatient, home care, and nursing home settings, including pre‑ and post‑bereavement phases.
Serve as a social work member of the hospice IDG and act as liaison between hospice staff and inpatient and nursing home staff.
Develop social work plans of care, establish goals, and make recommendations based on patient/family needs for patients directly admitted to inpatient units or nursing home/hospice residents.
Create discharge plans for patients admitted to inpatient facilities when appropriate.
Input clinical data proficiently and contribute to interdisciplinary team discussions.
Maintain communication and collaboration within an interdisciplinary team environment.
Qualifications
Master’s degree from an accredited school of social work.
Minimum of one year of Social Work experience in a healthcare setting.
Knowledge of Microsoft Word and Excel (preferred).
Valid New York State MSW licensure (LMSW or LCSW).
Valid New York State driver’s license; a vehicle may be required based on job location.
Ability to work within an interdisciplinary team environment.
