Duties Description Position DescriptionThis RN 2 will provide supervision to RN1s, clinical oversight and coordination of care for individuals receiving services through the Article 16 Clinic, ensuring that medical needs are addressed within an integrated, person-centered treatment plan. This position conducts health assessments, develops nursing care plans, monitors clinical progress, and collaborates with interdisciplinary team members to promote optimal health outcomes. The Nursing Case Manager also serves as a liaison between medical providers, clinic staff, and external healthcare resources to support continuity and quality of care. The candidate must be highly organized, professional and able to self-direct. Candidate must have good interpersonal skills, consistent communication with supervisor and be able to collaborate well with other RN’s and disciplines.
PRIMARY DUTIES AND RESPONSIBILITIES Duties will include
Under the direction of the Nurse Supervisor 1, the RN2 will provide oversight to subordinate nursing staff within the Article 16 clinic
Maintains and updates charts, records, and other documentation, including tracking necessary lab work, to ensure a high standard of care is provided to patients
Ensuring a patient’s plan of care meets their needs and acting as a liaison staff, providers, families or others involved in a patient’s treatment to facilitate modification of the plan as necessary.
Will complete appointment preparation for providers including preparing and reviewing documentation that will ensure nurses, providers and anyone else directly involved in the patient’s care have a clear understanding of the goals of the appointment.
Reviewing and processing new patient applications to ensure patients are seen in a timely manner relative to their treatment needs and maintaining and clinically triaging waitlists accordingly.
Assist with Camp Wilton prep and operation as it relates to medical services at the camp including reviewing medical needs of individuals attending camp, ensuring camp has an adequate supply of medical equipment and supplies to meet the needs of the individuals, and other relevant duties as assigned.
Will act as a liaison to the Dental, Psychiatry and Primary care departments to assist RN Supervisor 1 in determining the distribution of supporting duties among nursing staff.
Acts as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals.
Assist as directed in the orientation and training of new staff on duties and operations within the Article 16 Clinic.
Assisting in ensuring compliance with rules/regulations governing nursing practice in OPWDD.
Assist and ensuring implementation of facility policies and procedures related to nursing, including regular audits of performance of staff under their supervision.
Participate in various meetings and serving on committees as needed.
Attends/completes on-line mandated DDSO training, as well as discipline specific training.
Completes special projects as assigned.
Incident Investigation and other duties as assigned by Nurse Supervisor I
The Candidate must be able to perform CPR compressions on a supine individual on the floor. The Nurse demonstration of CPR compressions on the floor would ensure nurse ability to perform in worst case scenario, since we use no backboards in OPWDD.

Qualifications Applicants must possess a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. Substitution A bachelor’s degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master’s degree in nursing may substitute for two years of general post-licensure clinical nursing experience.
Transfer Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.
