How you’ll contribute

Facilitates the patient’s continued recovery after discharge. Provides aftercare plan coordination for patients who need ongoing medical/psychosocial care following discharge. In collaboration with other members of the healthcare team, assesses medical needs of the patient and offers suggestions regarding follow-up care options.

Qualifications and requirements

Bachelor’s degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting.

Prior experience with psychiatric and chemical dependency patients.

Unencumbered clinical license if eligible/needed per state regulations preferred. CPR certification and Crisis Prevention Training (CPI) within 30 days.
