What you’ll do:
Provide direct care and assist assigned individuals with accessing the community and increasing independence with communication, socialization, and daily living skills through teaching and rehabilitation methods. Develop and implement Individual Service Plan (ISP) components based on team member input and functional goals; submit ISP for review. Utilize effective/appropriate rehabilitation methods and strategies to encourage behavioral and functional skill growth and goal achievements. Conduct participant orientation, advise on participant program entrance/exit and program attendance. Plan and schedule internal and external activities and skill building opportunities such as volunteer experiences, guest speakers, or community outings in alignment with program participant needs and interests. Provide training to program participants around safety practices and program rules. Provide direct care and supervision of program participants while following individual service plans, encouraging goal achievement, and providing ongoing feedback to participants. Record and submit timely and accurate documentation while following confidentiality and HIPAA guidelines to meet funder and organization requirements. Ensure appropriate materials and supplies are requested or acquired to support planned, approved activities including maintaining petty cash and receipts. Assist in coordinating transportation and scheduling for activities. Performs other duties as assigned. What you’ll need:
High School Diploma or equivalent education required. Two years coaching or teaching others required along with proficiency using a computer and current technology such as Microsoft Office, a documentation database, and phone. Must be able to obtain First Aid/CPR certification. Good oral and written communication skills and have the ability to accurately document all observations, significant events, and services provided. Must be able to perform basic math. Access to reliable transportation for regular participation in work. Possession of valid Iowa Driver’s License, a good driving record, insurable, and able to provide proof of personal vehicle insurance with a minimum of $100,000 per person and $300,000 per accident auto insurance liability coverage. Ability to obtain a class D chauffeur’s License. Must be able to pass a criminal background check and dependent adult and child abuse background check. Ability to remain seated periodically. Ability to remain active for extended periods of time. Ability to regularly carry, lift, push, or pull up to 20 lbs regularly. Ability to tolerate variations in work environment temperature.
