Position Summary The RN Care Manager is responsible for assessing the care needs of participants, providing nursing and healthcare interventions, and evaluating care outcomes on an ongoing basis. In collaboration with the interdisciplinary team (IDT), the RN develops care plans tailored to meet participants' needs. This role involves delegating tasks to clinic, center, and homecare aides based on participant needs and care plans, and ensuring effective communication with the primary care provider and clinic staff.

Essential Duties and Responsibilities

Deliver high‑quality clinical care as an active member of the PACE interdisciplinary team (IDT).

Provide nursing care in various settings including centers, clinics, contracted facilities, and participants' homes in accordance with each individual’s care plan.

Participate in a 24/7 on‑call process for triaging participants’ needs.

Assess, plan, and coordinate participants' home care services.

Monitor participants’ acute and chronic care needs across all settings.

Ensure timely follow‑up on specialist visits and assist with obtaining necessary reports and documentation.

Monthly reconcile facility medication administration records for accuracy.

Notify participants about normal test results promptly.

Conduct timely and accurate nursing assessments in compliance with policies and regulations.

Implement nursing‑related care plan interventions.

Educate participants, caregivers, and families on self‑care and healthy lifestyles.

Review and revise goals in collaboration with participants and the IDT.

Foster positive relationships and communication with co‑workers, participants, and families.

Attend all interdisciplinary team meetings.

Assist team members in understanding nursing and functional needs related to participants' health issues.

Act in the capacity of a Home Care Coordinator as needed.

Engage in utilization review meetings and quality improvement initiatives.

Evaluate the competence of CNAs and Home Care Aides.

Participate in family meetings, in‑service training, and orientation programs as necessary.

Adhere to all PACE Program Policies and OSHA guidelines.

Maintain confidentiality of sensitive company and participant information.

Follow PACE Infection Control protocols.

Perform additional duties as assigned.

Experience, Education, and Certifications

Bachelor of Science in Nursing preferred.

State RN License required.

Must have a valid state license from the New Jersey State Board of Nursing.

BLS certification required (must obtain within 90 days of hire).

1 year of experience working with a frail or elderly population preferred; training will be provided if necessary.

Experience in home care, long‑term care, or managed care preferred.

Must have at least 1 year of experience providing care as an RN.

Bilingual with fluency in Hmong required.

Pre‑Employment Requirements

Must have reliable transportation, a valid driver's license, and minimum state-required liability auto insurance.

Must be medically cleared for communicable diseases and have current immunizations before direct participant contact.
