Job Summary
The Department of Pharmacy has a challenging opportunity for an Ambulatory Pharmacist Specialist to join their team.
This role involves providing pharmaceutical care to patients of all ages within the Medical Center. The successful candidate will be responsible for routine daily operations in their designated area of responsibility, including clinical patient care assignments, educational instruction, and occasional staffing.
Responsibilities:
- Serves as a key member of interdisciplinary patient care teams ensuring quality patient care and problem resolution.
- Participates in policy development and protocol implementation supporting the delivery of high-quality pharmaceutical services.
- Liaises with colleagues from various disciplines to ensure seamless coordination between pharmacy staff and other healthcare professionals.
- Maintains knowledge of best practices related to medication therapy management (MTM) strategies enhancing overall treatment outcomes for patients suffering chronic conditions or complex needs requiring focused attention throughout different life stages such as children’s medication safety & self-management alongside common use among families leading effective general access towards available treatments while choosing long-term personal health value implementations maintain based guidelines integrated modifications adherence bridging prevention innovate protocols cutting-edge impacts positive work standards!
