Job DescriptionJob DescriptionWe currently have an opportunity for a Bi-lingual Chinese-speaking Registered Nurse in Brooklyn to make visits to patients' homes.
Key Functions:
Perform initial assessment of clients for admission to home care, reassessments, and post-hospital discharge visits Supervise and instruct PCA/HHAs Review medication Develop aides care plan, and MD orders in HHA Exchange Collaborate with medical providers, and other members of the interdisciplinary team Work closely with coordination department Performs patient/family education Discharge planning Qualifications:
