Job DescriptionJob Description About Us:
We are a Veteran owned and operated, patient-centered healthcare practice dedicated to providing compassionate, comprehensive care to the geriatric population. Our team emphasizes dignity, quality of life, and individualized treatment plans. We are seeking a skilled and caring Nurse Practitioner with experience in wound management and primary care who thrives in a collaborative, supportive environment. Military preferred but not required.
Responsibilities:
Provide comprehensive primary care services to geriatric patients. Assess, diagnose, and treat chronic and acute conditions in an outpatient setting. Perform wound assessments and deliver evidence-based wound care management. Collaborate with physicians and interdisciplinary team members to ensure high-quality patient outcomes. Develop and implement individualized care plans, focusing on both prevention and management of chronic conditions. Maintain accurate and timely documentation in electronic health records (EHR). We use ECW for charting and training will be provided. Educate patients and families on disease management, wound care, and wellness strategies.
Qualifications:
Current Nurse Practitioner (NP) license in Texas. Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP). 2+ years of experience in geriatrics, wound care, and/or primary care strongly preferred. Knowledge of advanced wound care treatments, including debridement, dressings, and infection control. Strong clinical decision-making, communication, and patient education skills. Compassionate, empathetic, and committed to improving quality of life for elderly patients.
Pay/Benefits: Starting salary at $100,000-$150,000 depending on experience and flexibility. 18 days of paid time off yearly, with more than average paid time off for holidays. Matching Medical, Dental, and Vision optional. Multiple incentive programs and multiple bonus opportunities. Looking for Primary Care with Wound Care experience. Position is mostly for Telehealth visits but will have in person visits as well. Home visits occasionally.
