(3 Salary will be based on whether Full-time or Part-time and also on experience, we will negotiate during the hiring process. Job Duties/Responsibilities Conduct comprehensive patient assessments, develop and implement individualized care plans, and evaluate patient progress. Provide skilled nursing care to patients in their homes, including wound care, medication management, and disease-specific interventions. Coordinate and supervise the care provided by Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs). Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive and coordinated care. Educate patients and caregivers on disease management, self-care, and prevention strategies. Maintain accurate and timely documentation using electronic health records, adhering to all applicable regulations and policies. Monitor patient outcomes and identify opportunities for improvement in care delivery. Participate in case conferences, staff meetings, and continuing education programs to maintain professional competency and promote evidence-based practice. Qualifications Current RN license in the state of Colorado, in good standing. Minimum 2-3 years of experience as an RN, preferably within a home healthcare or related setting. Strong clinical skills and knowledge of current nursing practices, including wound care, medication management, and disease-specific interventions. Excellent leadership and communication skills, with the ability to collaborate effectively within an interdisciplinary team. Experience with electronic health records and documentation procedures. Current Basic Life Support (BLS) certification. Valid driver's license and reliable transportation for travel to patient homes within the Denver metropolitan area.
