About the Role:

The Hospice Home Team - RN Case Manager plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses in the comfort of their homes. This position is responsible for coordinating and managing comprehensive hospice care plans that address the physical, emotional, and psychosocial needs of patients and their families. The RN Case Manager collaborates closely with interdisciplinary teams, including physicians, social workers, and chaplains, to ensure seamless communication and continuity of care. By conducting thorough assessments and monitoring patient progress, the RN Case Manager ensures that care goals are met while respecting patient dignity and preferences. Ultimately, this role aims to enhance quality of life and provide support during end-of-life transitions within the Sheboygan & Manitowoc communities. This is a salaried position working Monday - Friday, first shift hours, with a minimal on-call rotation. Minimum Qualifications: Current and valid Registered Nurse (RN) license in the state of practice. Minimum of 2 years of clinical nursing experience, preferably in hospice, palliative, or home health care settings. Strong knowledge of hospice care principles, symptom management, and end-of-life care. Ability to work independently and manage a caseload effectively in a home care environment. Proficiency in electronic medical records (EMR) and documentation standards. Valid driver’s license and reliable transportation for home visits within the Sheboygan & Manitowoc area. Strong communication and counseling skills to support patients and families through difficult conversations. Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification preferred. Responsibilities: Conduct comprehensive patient assessments to develop individualized hospice care plans. Coordinate and oversee the delivery of nursing care and symptom management in patients' homes. Serve as the primary liaison between patients, families, and the interdisciplinary hospice team. Monitor patient status regularly and adjust care plans as needed to address changing needs. Provide education and emotional support to patients and families regarding disease progression and end-of-life care. Document all patient interactions, care plans, and clinical updates accurately and timely in compliance with regulatory standards. Facilitate communication with healthcare providers to ensure continuity and quality of care. Participate in team meetings and contribute to quality improvement initiatives. Skills: The RN Case Manager utilizes clinical nursing skills daily to assess patient conditions, manage symptoms, and implement care plans tailored to individual needs. Effective communication skills are essential for coordinating with interdisciplinary team members and providing clear, compassionate guidance to patients and families. Organizational and time management skills enable the RN to handle multiple cases efficiently while ensuring timely documentation and follow-up.
