FIT Family Partner - Community Service Agency Base pay range: $19.00/hr - $22.00/hr. This range is provided by Child & Family Services, Inc. Under the supervision and direction of the Senior Family Partner, the Family Partner is responsible for support and advocacy to parents/caregivers. They are directly responsible to the Senior Family Partner and to the Program Director of the Community Service Agency. Responsibilities & Essential Functions
Assist the family in understanding the scope and services of the CSA/FIT team and the family’s roles and responsibilities in the program Engage the parent/caregiver in home and community resources designed to address one or more goals on the youth’s treatment plan Teach parent/caregiver how to network/link and secure necessary community resources and treatment providers; provide home and community mentoring and education to enhance problem-solving, parenting effectiveness, and communication Participate in the child’s Family-Based Intensive Treatment (FIT) team by developing clear and outlined goals Provide coaching in support of decision-making in both crisis and non-crisis situations Support the family by modeling/educating families on accessing community resources Maintain regular contact, primarily in-person, with guardians/youth as set and agreed upon by the family Create a comprehensive, narrative assessment of the family, in collaboration with the family, to highlight strengths, needs and cultural identities Deliver strength-based, culturally and linguistically appropriate services under the supervision of a Senior Family Partner Participate in a weekly Family Partner clinical supervision meeting facilitated by an independently licensed clinician Promote linkage with outpatient providers by assisting the youth and family in attending outpatient appointments; attend and support meetings related to youth’s behavioral health needs when requested and clinically appropriate Support and help the family with the education system by attending school meetings and developing school plans/IEPs; engage with courts, state agencies and other community organizations as needed Monitor the family’s readiness for transition and satisfaction with CSA/FIT services Maintain consistent communication with supervisor and team members Participate in ongoing program and agency meetings as required Remain up to date with all paperwork and documentation within the FIT program Participate in systems of care committee meetings Cross-train to cover other Family Partner roles and responsibilities under the CSA as needed Perform other duties as required Travel & Other Duties
Travel is required to provide services in different settings and to attend agency meetings, events, and trainings. Must have a valid driver’s license and reliable transportation that meets CFS Driver Policy.
