Life Skills Coach/Case Manager - Young Adult Independent Living Program. Intercept Health is a family-owned agency serving various mental and behavioral health, substance use, and foster care needs in Virginia. We offer a $3,000 sign-on bonus. Responsibilities
Actively engage with young adults, ages 17-21, who are preparing for successful transition into the community. Serve as a positive role model for residents to observe and learn socially effective values, attitudes, and behaviors. Support residents in developing money management skills, obtaining employment, completing their education as applicable, learning daily living skills, and exploring career development opportunities. Contribute to the formulation and implementation of the resident transitional living plan and assist with life skills learning, transportation, and administrative duties. Education and Experience
Bachelor's degree in any field plus two years of experience in providing case management services to children and families; OR a human services Bachelor's degree (Social Work, Psychology, Counseling, Special Education, etc.) with experience providing case management services to children/youth and their families preferred; OR a human services Master's degree (Social Work, Psychology, Counseling, Special Education, etc.) with a student placement providing case management services to children/youth and their families.
