Responsibilities Powerback offers a new approach to delivering recovery, rehabilitation, respiratory, and wellness services across the nation. The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population, in private residences or independent living facilities. The role allows evaluation and treatment in the most functional environment— the patient’s home.

The Physical Therapist assesses needs, develops and delivers physical therapy programs to facilitate rehabilitation, and identifies referrals to other disciplines to address comprehensive patient needs. They implement individualized therapy programs designed to restore, reinforce, and enhance performance, instructing and training patients and caregivers in essential skills for promoting independence and productivity. They direct activities to promote and maintain health. This position provides a flexible schedule and work environment in various settings.

Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.

Ensure adherence to department and patient treatment schedules, timely clinical and billing documentation, and work‑related meetings.

Attend and contribute to patient care, staffing conferences, and other related meetings.

Make referrals to other disciplines and agencies to address comprehensive patient needs.

Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with each patient’s needs and capabilities.

Adapt program and/or plan of care according to individual patient needs.

Promote a safe environment and enforce elimination of fire and safety hazards.

Order supplies and equipment as necessary.

Clean, maintain, identify, and report equipment repair needs.

Put patient service first: ensure that patients and families receive the highest quality of service in a caring and compassionate atmosphere recognizing individuals’ needs and rights.

Screen, examine, and evaluate patients—including history, systems review, and application of appropriate tests and measures—to establish a diagnosis, identify impairments, determine predicted improvement level and time required, identify precautions/contraindications, design a treatment plan, and complete documentation of services. Services include:

Therapeutic exercise such as aerobic conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility, gait and locomotion training, neuromotor development, relaxation, strength, power, and endurance training.

Functional training.

Manual therapy.

Airway clearance techniques.

Integumentary repair and protection.

Use of electrotherapeutic, physical agent, and mechanical modalities.

Health and wellness.

Education on chronic disease management.

Falls assessment and interventions to reduce fall risk.

Case management in the home and community.

Design, fabricate, fit and/or apply adaptive devices, and train patients in using orthotics and prosthetics.

Select appropriate activities suitable to the individual's physical and mental capacity, intelligence level, and interests.

Instruct patients in skills required for participation in mobility activities and evaluate progress, attitudes, and behavior related to rehabilitative potential.

Perform routine reexamination as needed to modify or progress the treatment plan.

Incorporate health and wellness programming into treatment plans.

Perform home and community mobility assessments, providing recommendations for safe mobility.

Perform other related duties as required.

Qualifications

Must have a valid Physical Therapy license in the state(s) of practice or proof of license eligibility.

Must have a Master’s degree in Physical Therapy.

Or have a Master’s degree in a Physical Therapy‑related field plus a Bachelor’s degree in Physical Therapy.

Or have a Bachelor’s degree in Physical Therapy plus five years of progressively responsible PT experience.

Degrees earned outside the U.S. must be evaluated as U.S. equivalent.

Must have a valid driver’s license or reliable, independent transportation to and from patients’ residences.

Posted Salary Range USD $45.00 - USD $61.00 /Hr.

Bonus USD $5,000.00
